  • Northwestern Medicine is seeking BE/BC Internal Medicine Physician(s) for full-time/part-time/per diem Hospital at Home opportunities within the Northwestern Health System.

    The Hospital at Home physician treats acutely ill patients in the home as a substitute for in-hospital care. The Physician will provide hospital-level acute care virtually and/or in a patient’s home and will be part of the NM Hospital at Home team including (depending on each patient’s needs) Registered Nurses, Paramedics, Advanced Practice Providers, Pharmacists, Case Managers, Social Workers, Home Health Aides, Physical Therapists, and other operational staff. The Physician will be the attending of record for patients admitted to the Home Hospital service.

    • Block schedule of 7 days (virtual, home rounding and hospital based)
    • Medical Staff privileges may be required at multiple Northwestern Medicine Hospitals
    • Virtual nighttime coverage may be required
    • Provides home visits, when necessary, within defined radius

    • EPIC electronic health record system
