Medical Director, House Calls Program

The Medical Director will be joining Oak Street Health at a critical time in the history of our company and of primary care. The opportunity for impact is large and growing as Oak Street is leading the transition from fee-for-service to value based-based care. The Medical Director will be responsible for leading, as a dyad partner with the Mobile Practice Manager, the in-home primary care provider teams in the specified geography.

Responsibilities include:

• Provide direct management, oversight, and coaching of the local team
• Lead development of programs (i.e., complex care coordination, chronic disease management, psychosocial support, etc.) throughout the division
• Collaboration with the clinical teams in the center, medical directors, and other partners outside of Oak Street Health.
• Visit patients in their homes up to 3 days a week alongside their provider colleagues.
• Providing timely and constructive feedback to providers so the medical group continues to improve performance for patients and colleagues
• Provide managed care experience/teaching to providers
• Development of clinical compliance policies and guidelines
• Represent Oak Street to external partners (e.g., vendors, payers, providers) and create relationships and partnerships that support our mission and our value-based model

Additionally, we see this leader being a key thought partner in providing feedback on our clinical model especially as it pertains to the At Home Program.
