Job Title: Physician Location: Mishawaka, IN Position Type: Full-Time

Salary: Starting at $220,000 annually (negotiable based on experience) Flexible Hours Available

Description: We are seeking a dedicated and skilled Physician to join our team in Mishawaka, IN. In this role, you will be responsible for coordinating proactive care for patients with chronic conditions, as well as identifying wellness and preventative services. You will collaborate closely with clinical staff to provide high-quality care while promoting a patient-centered approach. Flexible hours are available to support work-life balance.

Key Responsibilities:

• Utilize effective patient care plans with appropriate diagnostic procedures and therapies.
• Ensure continuity of care, including scheduling and tracking return visits and follow-ups for chronic conditions.
• Provide whole-person care, including referrals to specialists such as social workers, nutritionists, and mental health professionals.
• Maintain clear, concise, and accurate documentation of patient care.
• Educate patients about their diagnosis, treatment options, and preventive care.
• Supervise and provide consultation to advanced practice nurses and residents when needed.
• Participate in quality assurance activities and physician performance audits.
• Contribute to the development of practice guidelines and protocols.
• Attend required in-service meetings and contribute to community health education.

Required Qualifications:

• Graduation from an accredited medical school and completion of a residency program.
• Board certification and Indiana State Medical License.
• DEA certification and CPR certification.
• At least 1-3 years of experience as a physician preferred (but not required).
• Strong communication, organizational, and time management skills.
• Ability to work well in a team environment and maintain flexibility.
• Proficiency in basic office equipment and software (Microsoft Office, EHR).
