We have an exciting opportunity for a part-time or full-time Advanced Practice Provider delivering care in both post-acute/skilled nursing facilities and residential senior living communities, with an immediate need in the Pearland, TX area.

Requirements Include:

• Build and maintain a working relationship with facility administration
• Effectively develop relationships with SNF Administrative and Clinical Teams
• Act as a liaison between the SNF and Trumen Physicians
• Assist and Track Patients in Trumen Value Based Programs
• Must have a minimum of one year experience working as a Nurse Practitioner preferably in a Post-Acute Care setting.
