• Adult only, No Peds
• Number of Physicians:2
• Number of NPs: 3
• Available immediately
• Active IN State License
• Board Status: Board Certified or Board Eligible.
• Flu vaccination: Required or Need Medical or religious exemption.
• Covid vaccination: Required or Need Medical or religious exemption.
• Certifications: BLS-AHA issued.
•  Mon (8am) - Fri (4.45pm) position.
