· Support staff: Yes

· Trauma Level: 3

· EMR: Epic

Practice Setting:

• Inpatient/Outpatient

Requirements:

· Board Certification: Yes

· License required: Active WA License or IMLC

· Fellowship: Preferred

Credentialing Timeframe: 90 Days
