Our team of clinicians at Erie HealthReach Waukegan offers family medicine, internal medicine, obstetrics and gynecology, pediatrics, dentistry, and behavioral health care, including medication-assisted treatment and psychiatry. Our nurses and many other team members coordinate care and partner with patients to address social drivers of health. We have teams dedicated to supporting our patients and providers in areas such as referrals, insurance benefits, pharmacy services, and health education.

Requirements:
- MD or DO from accredited institution, licensed physician in Illinois
- Federal DEA license, Illinois Physician's license
- Board Eligible or Certified in Family Medicine
- Interest in and comfort with working with diverse patient population
About Erie Family Health Center
Motivated by the belief that healthcare is a human right, we provide high quality, affordable care to support healthier people, families and communities.
The story of Erie Family Health Centers began in 1957 with a compassionate physician and an elderly patient in need of health care. The physician was Dr. Robert Snyder, an internist at Northwestern University Medical School, and his patient was Carmela Jacob. Today, Erie is a regional health care resource serving more than 82,000 patients a year at 13 health centers spanning the west side of Chicago to Waukegan. These sites include four large primary care centers that offer integrated medical and dental health services, three additional large primary care centers, the region’s oldest and largest comprehensive teen and young adult health center, and five school-based health centers.
Although 60 years have passed since Dr. Snyder first began providing medical care for West Town’s elderly, our mission remains the same: to deliver high quality health care services to the Chicago region’s medically underserved residents with compassion, cultural understanding and respect—regardless of their ability to pay.
