  • Toiyabe Indian Health Project is actively seeking a Family Medicine Nurse Practitioner to join their esteemed team here in the CA area! The incumbent of this position must be familiar with the aims and objectives of the Project and express a genuine enthusiasm for its success

    Job Qualifications

    1. Current California R.N. licensure plus a certified Family Nurse Practitioner required.
    2. Cardiopulmonary resuscitation (CPR) certified or obtain within three months of hire.
    3. Must be sensitive and possess an awareness and keen appreciation of Indian traditions, customs, and socio-economic needs of the Indian community.

    Job Details

    1. Collect patient data base information by taking health and medical histories, performing physical examinations and constructing a list of patient problems, ordering appropriate laboratory tests.
    2. Institute referral and coordinate patient services to promote continuous comprehensive care at the primary, secondary and tertiary level.
    3. Incorporate into the plan of care, provisions for the preventive measures to improve health status based upon individual cultural, psychological, social and environmental factors.
    4. Provide information to the patient, to enable making realistic decisions in solving health and related problems and insure joint responsibility with the professional health team members for the outcome of the patient's care.
    5. Advise and counsel patients about normal developmental, maturational status and normal life crisis. Identify and intervene in problems related to these areas.
    6. Collect and record data according to the problem-oriented medical record.
    7. Provide support/counseling to patients with chronic disease in regard to psycho-social, environmental and physical well-being as well as monitoring condition.
    8. Institute emergency medical measures in life-threatening situations.
    9. Access and intervene in common developmental and behavioral problems of individuals in the family.
    10. Perform routine physical examinations and preventive health measures including immunizations, developmental screening and other common screening laboratory procedures appropriate to each age group.
    11. Diagnose and treat common health problems in a given population. Collaborate and consult with physicians to collect the data for less common and more complicated health problems. Work with other staff (physicians, counselors, CHRs, etc.) to establish, evaluate and monitor treatment programs.
    12. Counsel patients on available contraceptives and facilitate availability of indicated birth control measures.
    13. May dispense and administer drugs to the ultimate user in accordance with written standardized procedure(s) developed under these guidelines shared with in-house Pharmacist.
    14. Verbally transmit a valid prescription to a pharmacist, by telephone in person, or electronically.
    15. Assume duties as patient advocate between patients and health care systems.
    16. Respond to community request for health promotion and educational activities; knowledge of agencies and resources available to patients/staff for further evaluation, education, etc.
