Primary Responsibilities:

• Schedule: Monday – Friday
• Provide in home primary care services for our high-risk members.
• Ensure members are receiving high quality follow-up care (post discharge) in home setting.
• Mix of in person new visits, follow ups and virtual / telephonic appointments.
• Review of medication management, patient education, available resources.
• Educate members on topics such as disease process, medication, and compliance.
• Meet with patient and patient’s family when discussing hospice and palliative care.
• Advanced schedule coordinated by Medical Assistant

Required Qualifications:

• Master’s degree from four-year college and/or a professional certification beyond a four-year college.
• Certificate of completion from Master of Physician Assistant Studies program or accredited Program
• Current NCCPA or ANCC and/or AANP Certification required (Family Medicine, Internal Medicine, Adult Gerontology, Acute Care)
• Unrestricted / Active CA PA license
• Current CA DEA certificate required prior to start date
• 1+ years of experience as practicing APC
• Active BLS certification
• EMR Proficient
• Excellent patient care and time management skills
• Work independently and without direct supervision
• Ability to travel to multiple locations within designated region

Preferred Qualifications:

• 1+ years experience; home health care or home visits program
• Working knowledge of managed care, health plans, medical groups
• Working knowledge with risk adjustment, coding

California Residents Only: The salary range for this role is $114,784 to $160,000 per year. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
