Job Highlights
Full-Time Nurse Practitioner
Reports To: Chief of Internal Medicine
Responsibilities:
- Involve with direct patient physical assessment, education, and health planning
- Maintain continuity of care by regular patient follow-up
- Assist in the coordination of services with other disciplines and patient's insurance plans
- Advocate for patient in arrangement of medically necessary services
- Communicate with community's resources to ascertain adequate assistance for patients in need
- Refer patients to specialty services when necessary
- Consult with collaborating physician and other members of the health care team to establish plan of care
- Participate in quality assurance process to assure optimal care delivery
- Provide comprehensive case management services that include emphasis on health promotion and disease prevention
- Monitor patient outcome in accordance to the practice protocols
- Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning
- Act a resource person for the nursing staff
- Conduct education in service on practice issues
- Perform other duties as assigned
Minimum Qualifications:
- Graduated from approved Nurse Practitioner Program
- Licensed as Nurse Practitioner in NYS
- Basic CPR (BCLS) certification required
- At least 1 to 2 years of clinical experience as a primary care Nurse Practitioner
- Board/Specialty Certification preferred
- Knowledge of a second language (such as Bangla/Bengali, Mandarin, Cantonese, Korean, Spanish) preferred
Compensation and Benefits:
- Salary: $105,000 - $115,000 per year with productivity-based incentive pay. Experienced providers can expect to make more annually.
