Our mission starts with assisting rural middle Georgia with chronic care management. Chronic care management is healthcare beyond the traditional provider office visit. Our model of care encompasses a holistic approach. Holistic care is understanding and managing the lives of our members, beyond physical treatment. We started the Healing Hearts Elevate Energy, Listen & Learn to Promote Wellness Campaign because of several communities requests for help.
