Summary

• Dr. Sarah Alsgaard is a board-certified Psychiatric Mental Health Nurse Practitioner specializing in the diagnosis and medication management of mental health disorders. She graduated with a Bachelors of Science in Nursing from the University of Wyoming in 2011 and practiced as a registered nurse in specialties including acute cardiac & pulmonary care. Following she attended graduate school at George Mason University in Fairfax, Virginia earning a Doctorate in Nursing Practice in 2017. She relocated to Durango, Colorado and has practiced in psychiatric care across the lifespan in both inpatient & outpatient settings. Sarah also works with the local community mental health center Axis Health System. She is the provider for the assertive community treatment (ACT) team, a community based and intensive interdisciplinary care program for the chronic and persistently mentally ill. She is also the provider for the Ascent Colorado program, focusing on early intervention and community based services for adolescents and young adults experiencing a first episode of psychosis.
