Summary

• I am both a board-certified Advanced Practice Registered Nurse and Psych Mental Health Nurse Practitioner [PMHNP]. I conduct and record patients' past medical history on plan members; and formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings from the assessment. I also review current symptoms and identify diagnoses to be used in care management and active medical management of treatment. I perform physical examinations that include A1C tests, diabetic retinal eye examinations, monofilament tests, foot exams, medication reviews, depression, CAGE, Peripheral arterial disease screening [PAD], and nutrition screening. I educate patients on topics such as disease process, medication, adherence/compliance, and diet and exercise. I comply with all HIPAA regulations and maintain the security of protected health information (PHI).
  As a Psych Mental Health Nurse Practitioner, I assess, diagnose, and treat the mental health needs of patients. I also prescribe medications for patients who have mental health disorders; develop appropriate treatment plans and provide effective management of patient care with proper evaluation.
