Summary

• I am a clinical pharmacist, working as part of an integrated care team providing care in the primary care setting for geriatric patients. I work with a team of providers, care coach RNs, behavioral health specialists, and social workers to focus on holistic care for one of our most vulnerable patient populations. I help to support the providers with making appropriate medication recommendations as well as help provide direct patient management for chronic care with diabetes, CHF, COPD, and transitions of care for our patients at the highest risk for complications.
