  • CEO of CCRx, a team of clinical pharmacists, which provide preventative services and chronic disease management to Medicare patients. Our team specializes in Medication Therapy Management (MTM), Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Annual Wellness Visits (AWV), and Transitions of Care (TOC). We utilize innovative methods to provide personalized services to our clients making the workflows seamless and creating less of a burden on the providers.
