Summary
- I am a Critical Care Nephrologist.95% of physicians and 99% of nurses don’t know this specialty exists!! In a nutshell it has taken us nephrologists 70 years to figure out that in the “Cardiovascular System” ( which consists of the heart, all the arteries, veins capillaries and lymphatics ) are “controlled” 100% by the kidneys; and the "heart" is just a dumb pump: because all it does is pump. It has no brain.
So we know now that “the brains” of the “Circulation are in the kidneys. Each kidney is a massive micro-computer consisting of 1 million microtubules called “nephrons” each of which is a complete microcomputer with a motherboard, microchips, sensors along with an amazing operating system like Windows or OS Sierra called Rennin-Angiotensin. Each microchip resides in the “ juxtaglomerular apparatus “https://en.wikipedia.org/wiki/Juxtaglomerular_apparatus. Each nephron is as powerful as an iPhone. They all work independently to maintain “the Circulation”, every minute birth to death. The formula that controls the Circulation is P=VxR (Pressure= Volume x Resistance), a variation of Ohm’s Law.
Thus Critical Care Nephrology (CCN) is the specialty best suited to handle the circulation.Amazingly both Nephrology as well as CCN where discovered in the 1950s by Harvard Cardiologist Prof.John Merrill MD http://cjasn.asnjournals.org/content/4/1/2.full. He also performed the world’s first kidney transplant at Harvard in 1954.In this article you will read about Prof.Franklin McDonald MD whom Merrill trained and he later trained me which is when I first heard the world "Critical" in 1974.Merrill trained every academic nephrologist running all 50 States Nephrology programs.
There are now several 1 year CCN Residency Programs open to either a trained Intensivist or nephrologist and in the next 3-4 years ABIM will have a Board exam. Here is the Core Curriculum of this new Critical Care Nephrology Board exam: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2697970/
