Summary

  • A native of Stockton, California, I graduated from College of the Pacific and Stanford University. I received my medical degree from Dartmouth Medical School and I completed my training in internal medicine, as well as additional training in cardiology, at Yale-New Haven Hospital.

    Currently, I am leading a newly created clinical program called Intensive Outpatient Care (IOC) program at NorthBay Healthcare System in northern California. Our unique program provides care for a panel of approximately 300 patients who have multiple and highly complicated medical, surgical, psychiatric, and social issues. Most of our patients are more than 65 years old and have coronary heart disease, cardiac arrhythmia, diabetes, hypertension, chronic kidney disease, lipid disorders, pulmonary disorders, complicated headache/migraine, and depression/anxiety. Some of our patients were referred to us by primary care providers or medical and surgical specialists at NorthBay. A number of patients came to us by referral from our hospitalists, case management teams, as well as from our established patients. Our ultimate goal is to provide highest possible quality of care and best possible experience and result for our patients, especially those patients who have history of multiple admissions to the ER/hospital for various reasons. Our unique program has been able to function at or near the level of internal medicine sub-specialists (cardiology, nephrology, endocrinology...). However, when needed we also refer our patients to appropriate specialists and sub-specialists both inside and outside NorthBay (Stanford, UCSF...)

    My personal and professional interests are to innovate and develop care models which help achieve the best possible clinical and financial outcomes for patients, our institution and society.
