Summary
- My work focuses on the development of health care system redesign to improve population health outcomes. This pursuit offers a great opportunity to combine my experience in managing teams of health care providers, public health, quality improvement, health policy, managed care, and systems thinking.
Health Quality Partners (HQP) is the only care coordination program out of the 15 tested in the CMS Medicare Coordinated Care Demonstration that was studied as a randomized prospective trial for over 12 years - because of its success in improving health outcomes and lowering Medicare expenditures. For over 5 years a similar model has successfully served and reduced health care costs among higher-risk Medicare Advantage beneficiaries enrolled in Aetna health plans.
HQP is now helping ACO's replicate this model through design, implementation support, training, and an integrated IT platform designed for this field-based work. Learn more at www.hqp.org
