• In palliative care work, we help persons and families to navigate the healthcare system, which reduces stress and anxiety. Though listening to the person's story of what is important, our team is able to develop a plan of care that uplifts quality of life and infuses hope. Additionally, we offer expert symptom manage and have goals of care conversations about how they want to receive care over their lifetime (Hoping for the Best, Planning for the Worst). Palliative care is treatment for any age or stage and people often are seeking curative or medical management treatments for their chronic illness. We are an added overlay of support to the primary care team and can help increase communication between person, family and healthcare team. We help healthcare systems enact change models and assist in 30 day readmission reduction.
