Though listening to the person's story of what is important, our team is able to develop a plan of care that uplifts quality of life and infuses hope. Palliative care is treatment for any age or stage and people often are seeking curative or medical management treatments for their chronic illness. We are an added overlay of support to the primary care team and can help increase communication between person, family and healthcare team.
