Summary

  • In Medicine, assuring the best outcomes for patients and their communities must take priority over administrative, regulatory, and profit concerns.

    Limiting the implicit biases toward alĺ forms of mental illnesses including substance use disorders needs to be a priority.

    How we pay for and incentivize clinical care is dysfunctional and helps explain why we have arguably the worst outcomes of any country as it relates to cost effective outcomes, let alone our relatively poor outcomes. We must as a profession recognize the significance of social determinants as predictors of health outcomes.

    Third party influence on clinical care must be less. Good medical care is always individualized and contextual variables are not efficiently addressed by third parties especially when they have no accountability for poor outcomes and may sometimes even deny standards of care. Healthy authority implies accountability. The accountability for poor outcomes by third parties is sorely lacking. The limited accountability that exists is primarily financial and that is not adequate if trust is to be furthered. Without trust, cost-effective outcomes are compromised.

    If someone wants to hire me I have inordinate expertise and outcomes related to managing opioid use disorders and comorbid conditions in an outpatient setting. I am interested in leveraging this expertise.

    I naturally reason abductively and recognize system problems better than most and am most passionate about improving them.
