Heather Hodnett currently practices in Phoenix, AZ, as the Director of Palliative Care for Savior Palliative Care. She has differentiated herself as an emerging leader in program development for home-based palliative care, helping companies drive billable income in this much-needed yet underdeveloped area of medicine. Heather has experience working with adult and geriatric populations in primary care, transitional care, palliative care, and hospice across many settings, including outpatient, long-term care, post-acute/SNF, group home, and independent home settings. Having grown up in Oregon, she found her way to Phoenix by way of Nashville, TN, where she completed her Nurse Practitioner program at Vanderbilt University School of Nursing.
