Summary

• Coastal Geriatrics provides primary medical care to older adults in the Santa Barbara community. We contract with Medicare, MediCal, and all other major health insurance plans for patients aged 65 and older. We create individualized care plans for each patient. We complete geriatric assessments including wellness visits, cognitive evaluations, falls risk assessments, functional assessments, safety care plans, and goals of care discussions. We specialize in frailty and dementia care, and provide additional help in these areas. Coastal Geriatrics will optimize wellness, function, independence, and dignity of each patient while providing comprehensive geriatric medical care.
