Summary
- Dr. Croak received his D.O. degree from Midwestern University’s Chicago College of Osteopathic Medicine in 1996, his traditional rotating internship at St. Vincent’s in Toledo, OH in 1997, and his OB/GYN residency at Grandview/Southview Medical Centers in Dayton, OH and graduated in 2001. Dr. Croak completed a 3 year fellowship in Female Pelvic Medicine and Reconstructive Surgery at the Mayo Clinic in Rochester, MN until 2004. He was awarded 1st place for the Berlex Excellence in Poster Presentations at the 70thannual ACOOG conference. He successfully defended his thesis regarding “Questionnaire-based Characterization of Urinary Symptoms,” and was awarded a Master’s in Biomedical Sciences in 2007 from Mayo Clinic. Dr. Croak began private practice in 2004 by founding the Northwest Ohio Center for Urogynecology & Women’s Health, working alongside his wife & his father, Dr. James Croak. He has published numerous peer reviewed articles and has taught OB/GYN residents as an Associate Professor of FPMRS for over 15 years from the Ohio University Heritage College of Osteopathic Medicine. There, he has received the Mentor of the Year award twice and was inducted into the Ohio Osteopathic Mentor Hall of Fame in 2015. He was the first Gynecologic Surgeon from Northwest Ohio to gain admission to the prestigious Society of Gynecologic Surgeons, & is also a current member of the American Urogynecologic Society, American Association of Gynecologic Laparoscopists, and American College of Osteopathic OBGYN’s, where he recently was inducted as a Distinguished Fellow of the College in 2018. He sits on the National Board of Examiners for his subspecialty. He is the Gynecologic Surgical Department Chair at St. Luke’s Hospital. He is past President of the Toledo OB/GYN Society and has been nominated as a Toledo Hometown Healthcare Hero. He has received 2 national consumer awards for excellence & compassion in practice. Dr. Croak is married with two sons and resides in the Toledo area.
