1. Practice Setting: Outpatient Pediatric Office
2. Reason for Coverage: Provider Vacancies & Maternity Leave
3. Dates of Coverage: ASAP-4/30/25
4. Schedule: Weekdays, Mon-Fri, 8-5pm
5. Call Schedule: No Call
6. Call Ratio: No Call
7. Average Number of Well Check Visits Per Day: 24 visits scheduled daily (20 min), with 4-8 well child visits per day on average
8. Number of Physicians in Practice: 3 part-time
9. How Many Mid-Levels Provide Coverage: 0
10. Will the Locums be Responsible for Supervising Other Health Care Providers: No
11. Number of Exam Rooms: 6
12. Describe Office Equipment: POC: flu, strep, covid, lead, hearing, vision, etc.
13. Describe Support Staff: Full interprofessional pediatric primary care team including nurses, community health workers, and medical assistants.
14. Average Number of Patients Per Day: 24 scheduled, likely 20 seen considering no shows/late cancellations
15. Documentation System/EMR: Meditech Expanse
16. Required Pediatric Procedures: Outpatient Pediatrics, Routine outpatient pediatric primary care

Submission Requirements:

1. Board Certified Pediatrician or Family Practitioners - REQUIRED
2. Active MA License - REQUIRED
3. BLS/ACLS - REQUIRED
4. Prefer local but open to consider applicants with open availability during period of need with added expenses.
