At SSM Health, we are committed to delivering exceptional healthcare services. Our Fond du lac Regional Clinic, a 110-physician multispecialty group serving east-central Wisconsin, is seeking a Family Medicine APP to join its main clinic location.

Job Details:

• Full-time opportunity, 0.8 FTE (32hrs/week)

• Shared panel with physician

• Great work/life balance!

• Clinic is open Monday-Friday, no nights, no weekends no holidays!

• Clinic located on main hospital campus

• Shared Call: entire Fond du Lac region is covered together

• 1 night a month
• 2 weekends a year

• $5,000.00 yearly stipend for call rotation

• EPIC EMR

Minimum Qualifications:

• Graduate of nationally accredited Physician Assistant (PA) Program
• Ability to obtain PA licensure in Wisconsin

Facility Description - St. Agnes:

This APP will be practice sharing with the current Physician. The Physician is the Medical Director and is only in clinic 2 days a week.
