OptumCare, AZ is a multi-specialty group of physicians, Nurse Practitioners, and Physician Assistants, 4 primary care locations. OptumCare, AZ offers patients compassion, innovation, and quality care throughout the Tucson area.
Primary Responsibilities:
- Acts as the APC liaison to the adult medicine leadership team, championing views and concerns of the APC role; promotes a collaborative relationship between physicians and APC’s
- Participates in developing and implementing programs to increase APC engagement
- Participates in developing and implementing organization specific initiatives around coding, documentation, STAR, HEDIS, and PCMH
- Participates in monthly meetings with physician and operations leadership
- Participates in various committees as assigned
- Provides diagnosis, treatment, therapy and management of patients according to current standards of care utilizing Best Practices recommendation
- Utilizes evidence-based care processes for cost-effective and efficient use of lab, radiology and extended service providers, including specialists, hospitals, skilled nursing providers, and other ancillary services
- Collaborates with physician and seeks direction and advice on any complex issues or cases; Identifies and directs complex cases to physician for case handling and disposition if unable to stabilize conditions
- Create appropriate referrals for specialty care needs
- Provide guidance to nursing personal on treatment plans and care coordination
- Maintains knowledge of STAR, HEDIS, and medical coding and diagnosis requirements
- Promotes the organizational culture
Required Qualifications:
- Active Arizona Nurse Practitioner or Physician Assistant License and be in good standing
- Ability to obtain DEA licensure/prescriptive authority post-hire in states where applicable
- Nurse Practitioner Certification from either the ANCC, AANP or AACN as Adult, Geriatric, Family or Acute Care NP or certified Physician Assistant
- 1+ years of clinical experience in highest level of education
- Computer literate
Preferred Qualification:
- Proven excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families
