HarmonyCares is the nation’s largest in-home value based primary care provider. We operate across 14 states with 37 medical groups in primary care. All of our care services focus on the geriatric population in the home setting. We have multiple divisions of business – Primary Care Medical Group, Home Health, Hospice, and In Home Assessments. We see high acuity patients within a radius of our Medical Group office in a company car.

The Physician Leader delivers primary health care and patient care management in a residential setting and has clinical oversight of Advance Practice Providers.

Qualifications:

• Experience in Geriatrics or complex patients preferred
• Active/unrestricted medical license and DEA
• Board Certified or Board Eligible
• EMR experience
