      • Full primary care (physicals, chronic condition management, acute episodic care, full lab services, immunizations, shots)

        • Minor acute care

      ** Schedule**

      • Monday, Tuesday, Thursday, Friday; 7a-6p (one day will be 8a-7p)

        • No call

      ** Requirements**

      • Certified Family Nurse Practitioner
      • BLS and DEA certifications required
      • Indiana medical license
      • Minimum of 36 months of experience

      $75/hour
