Summary
- LifeScape Medical Associates and LifeScape Premier (www.lifescapearizona.com) were created with a mission to INSPIRE HEALTH. We were one of 15 facilitated practices in the AAFP TransforMed National Demonstration Project to evaluate the patient-centered medical home model. LifeScape has garnered national recognition including cover feature on Medical Economics for our healing office design principles and meeting with President Obama on healthcare reform. We aim to restore health rather than just "manage chronic disease" and are achieving phenomenal results with the application of functional medicine - targeting and eliminating the root causes of chronic disease. Type 2 diabetes is curable, as are many mood disorders. It is no longer acceptable to simply throw prescriptions at patients or, as one patient summed up her previous care, "monitor patients deterioration." LifeScape is a model for what primary care can and should be.
