Summary

  • Jay W. Lee, MD, MPH, FAAFP, is a member of the board of directors of the American Academy of Family Physicians. The AAFP represents 130,000 physicians and medical students nationwide. As an AAFP board member, Lee advocates on behalf of family physicians and patients to inspire positive change in the U.S. health care system.

    Lee is currently Medical Director at Integrated Health Partners of Southern California, a clinical integrated network serving 400,000 patients whose medical home is at one of nine community health centers in three counties. He is an associate clinical professor of family medicine at the University of California, Irvine and continues to see patients at a local community health center. Lee has served in various physician executive roles during his career: leading practice transformation at a vertically integrated health system, innovating primary care model at a national value-based care organization, and overseeing day-to-day operations at nationally recognized community health centers in Los Angeles and Orange counties.

    A member of the AAFP since 2002, Lee has served as a delegate to the Congress of Delegates and as a member of the Commission on Health of the Public and Science. Previously, Lee was a member of the Practice Transformation Tactic Team and the Health Equity Cross-Tactic Team for Family Medicine for America’s Health and served as core faculty for AAFP’s Leading Physician Wellness program. He also served as the conference convener and minority co-convener for the National Conference of Special Constituencies and was a member of the inaugural class of AAFP media ambassadors.

    Lee earned his bachelor’s degree in human biology from Stanford University. He earned his medical degree from the Keck School of Medicine of the University of Southern California and completed his residency at the Memorial Family Medicine program in Long Beach, CA. Lee also earned a Master of Public Health from Harvard School of Public Health in Boston, MA.
