Family Nurse Practitioner – Outpatient Primary Care | Erie, PA

Summary: Community Health Net is seeking a compassionate and dedicated Nurse Practitioner to join our growing team in providing comprehensive primary care services to patients of all ages. This includes diagnosis, treatment, preventive care, health maintenance, and care coordination in an outpatient setting.

Qualifications:

• Valid and current Pennsylvania Nurse Practitioner and Registered Nurse licenses

• DEA license (preferred)

• Completion of credentialing requirements

• Experience in an outpatient or community health setting preferred

• Familiarity with Electronic Health Records (EHR), ideally NextGen

• Commitment to providing high-quality, patient-centered care with compassion and respect
