Lead and Innovate: PACE Medical Director Opportunity in Norfolk, VA

Sentara Medical Group has a full-time opening for a Medical Director to join our Sentara PACE (Program for All-inclusive Care for the Elderly) team located in Norfolk, VA.

We are looking for a Family Medicine/Internal Medicine Physician to lead an interdisciplinary team to manage a small panel of 150-500 elderly patients, focused on quality and clinical outcomes. We have two innovative practice models, our Norfolk Comprehensive Care Clinic and our Norfolk PACE program. The ideal candidate will have a background in value-based care.

PACE is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative to nursing facility eligible individuals who would prefer to remain at home with services coordinated by Sentara PACE. The program provides supportive care for participants, including comprehensive medical and rehabilitative services, in-home services and transportation.

Position Highlights:

1. Competitive salary with bonus opportunity
2. Clinic duties include providing primary care to patients 55+ at a low-volume clinic with typical volume of 6-10 patients a day
3. Occasional home visits or visits to assisted living facilities as needed
4. 8-hour workday, generally from 8am-4pm, with rotating call schedule every three weeks, Monday 5pm-Monday 8am
5. APP on-site for daily patient management that works alongside physician to manage patients and do recerts
6. Team-oriented and collaborative environment with interdisciplinary team meeting for one to two hours most days

Minimum Requirements:

1. M.D. or D.O., licensed to practice in Virginia
2. At least three years of experience as a practicing physician
