Outpatient Geriatric Primary Care in Daytona Beach

Conviva Senior Primary Care is looking for a new Primary Care Physician to our team in Daytona Beach, Florida.

About the role:

• Monday-Friday, 8-5 Hours in an Outpatient Setting
• See 12-15 patients per day under a value-based care model
• Team-based model with daily huddles
• Full support staff including care coaches, MAs and APPs
• eCW EMR system

Benefits:

• Base salary plus 20% quality incentive pay, for total earnings up to $300,000
• Sign-on bonus, relocation offered in some circumstances

Requirements:

• BC/BE
• Florida Licensure
• Experience with geriatric primary care
