Sentara Community Care Associate Medical Director position open in Hampton Roads

Sentara Community Care was founded with the goal of developing innovative care models that not only address but also help reduce health disparities in under-resourced communities. Since its inception in 2022, the program has grown rapidly in both scope and reach. We are seeking an experienced Physician to serve as an Associate Medical Director/Clinician across our Hampton Roads locations splitting time between Norfolk and Newport News, VA.

Position Highlights:

• Location: Newport News and Norfolk, VA
• 0.9 FTE Clinical, 0.1 FTE Associate Medical Director
• Experience treating family medicine, pediatrics, and adults with complex issues
• Shared call schedule, with RN first call
• Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients’ health holistically.
• Review quality and outcome metrics monthly with the healthcare team
• Collaborate with local civic and faith-based leaders on community education programs
• Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
