Primary Responsibilities:

  • Optum Clinic Locations: All of the lower San Gabriel Valley, Los Angeles area
  • Complete senior wellness visits at company-owned outpatient facilities and independently contracted physician offices, as well as some home/custodial visits
  • Schedule: Monday - Friday
  • Average Daily Patient Census: Up to 16 patients per day (10-12 patients per day on average)
  • We will consider and train new graduates

Required Qualifications:

  • Master’s degree from a four-year college and/or a professional certification beyond a four-year college
  • Certificate of completion from an accredited Advanced Practice Clinician Program
  • Unrestricted California Advanced Practice Clinician license or in process of obtaining
  • Current AANP/ANCC/NCCPA certification
  • Current California DEA certificate required prior to start date
  • EMR Proficient
  • Excellent patient care and time management skills
  • Ability to work in a fast-paced environment, team player, adaptable to changing priorities

Preferred Qualifications:

  • Previous HCC experience
  • 1+ years of previous managed care setting experience
  • Fluent or proficient Medical Spanish

California Residents Only: The salary range for this role is $120,000 to $170,000 per year. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
