Lead the Opening of our Primary Care Clinic

Conviva Senior Primary Care in Wesley Chapel, Florida is looking to hire an experienced Primary Care physician to lead the opening of our newest location in Wesley Chapel, Florida.

About the role:

• Monday-Friday, 8-5 Hours in an Outpatient Setting
• See 12-15 patients per day under a value-based care model
• Team-based model with daily huddles
• Full support staff including care coaches, MAs and APPs
• eCW EMR system

Benefits:

• Base salary plus 20% quality incentive pay, for total earnings up to $300,000
• Sign-on bonus, relocation offered in some circumstances
• Occurrence-based malpractice

Requirements:

• Florida Licensure
• Experience with geriatric primary care/value-based care
