Position Highlights:

  • Schedule: Monday – Friday
  • Office Location: Irvine
  • Opportunity to be integral part of new program focused on health and well- being and the quality of care provided to our Medi-Cal members.
  • Responsible for connecting with members to complete telehealth appointments; transition of care including recent discharge from ER, follow up appointment with member’s PCP, behavioral health appointments, accessing community resources, medication reconciliation. Responsible for the coordination of care with specialists and appropriate ancillary services.
  • Complete all documentation and paperwork in a timely manner. Maintain quality of standards defined by the medical group.
  • Age Range of Patients: 13 years of age and older.
  • Appointment Matrix: 15 minutes; average of 30 patients per day.
  • Focus on quality driven metrics and Medi-Cal Pay for Value (P4V) performance measures.
  • Participate at occasional health fairs as scheduled for members.

Required Qualifications:

  • Master’s degree from four-year college and/or a professional certification beyond a four-year college.
  • Certificate of completion from Master of Physician Assistant Studies program or accredited Nurse Practitioner Program
  • Current NCCPA or ANCC and/or AANP Certification required
  • Unrestricted / Active CA PA or NP license
  • Current CA DEA certificate required prior to start date

Preferred Qualifications:

  • Fluency in Spanish
  • Minimum 1-2 years’ experience – outpatient setting - primary care, FQHC, managed care
  • Working knowledge of risk adjustment, quality metrics
