    Key Responsibilities:

    • Conduct in-home visits, health assessments, and care planning for members with acute and chronic conditions
    • Collaborate with other healthcare providers to deliver high-quality, patient-centered care
    • Participate in call rotation of telephonic on call coverage after hours

    Requirements:

    1. Bachelor’s in nursing with Wyoming APRN license (Master’s preferred).
    2. Board certification for Nurse Practitioners and DEA licensure.
    3. Valid driver’s license and proof of insurance.
