    Reports To: Chief of Internal Medicine

    Responsibilities:

    • Involve with direct patient physical assessment, education, and health planning
    • Maintain continuity of care by regular patient follow-up
    • Assist in the coordination of services with other disciplines and patient's insurance plans
    • Advocate for patient in arrangement of medically necessary services
    • Communicate with community's resources to ascertain adequate assistance for patients in need
    • Refer patients to specialty services when necessary
    • Consult with collaborating physician and other members of the health care team to establish plan of care
    • Participate in quality assurance process to assure optimal care delivery
    • Provide comprehensive case management services that include emphasis on health promotion and disease prevention
    • Monitor patient outcome in accordance to the practice protocols
    • Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning
    • Act a resource person for the nursing staff
    • Conduct education in service on practice issues
    • Perform other duties as assigned

    Minimum Qualifications:

    • Graduated from approved Nurse Practitioner Program
    • Licensed as Nurse Practitioner in NYS
    • Basic CPR (BCLS) certification required
    • At least 1 to 2 years of clinical experience as a primary care Nurse Practitioner
    • Board/Specialty Certification preferred
    • Knowledge of a second language (such as Mandarin, Cantonese, Bengali, Korean, Spanish) preferred

    Compensation and Benefits:

    • Salary: $105,000 - $115,000 per year with productivity-based incentive pay. Experienced providers can expect to make more annually.
