WellMed, part of the Optum family of businesses, is seeking a CareCoach Connect Nurse Practitioner or Physician Assistant to join our team as a Texas Statewide Traveler. Optum is a clinician-led care organization that is changing the way clinicians work and live.

This role primarily involves travel to locations within a three-hour radius of the Houston metropolitan area, with overnight stays required approximately 25% of the time. However, there may be expected trips to areas in East or West Texas which will require stays of one week. All travel expenses are fully covered. The role offers a base salary along with multiple incentives.

Required Qualifications:

  • Graduate of an accredited Nurse Practitioner or Physician’s Assistant Program
  • Current Licensure through the State Board of Nurse Examiners or State Medical Board Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
  • Current prescriptive authority from the State Board of Nursing or State Medical Board
  • Comfortable with chronic condition management for Medicare-aged population
  • Must have a valid driver’s license within the state of work
  • Ability to establish effective working relations with patients and their families

Preferred Qualifications:

  • 2+ years internal medicine/family practice with geriatric care experience
  • Knowledge of Palliative care and end of life symptom management
  • Bilingual (English/Spanish) language proficiency preferred
  • Ability to assess the physical, and psychosocial, status of geriatric populations
  • Ability to work independently and interdependently with the interdisciplinary team, showing initiative and motivation
