Primary Responsibilities:

  • Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
  • Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care – both in the home and virtually - thereafter, including visits to address changes in condition
  • Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization
  • Consults with the Landmark Physician Leaders and coordinate with PCP in these decisions as available
  • Comfortable with basic procedures of episodic nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, basic wound care, etc. New hire and annual skills training provided
  • Performs post discharge and urgent visits to ensure patient stability is consistent with their goals of care
  • Participates in HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
  • Covers assigned geographic territory. May require substantial drive time in more rural areas
  • Join morning huddles on days worked
  • Responsible for reviewing updates to clinical and operational workflows when needed

Required Qualifications:

  • Graduate of an accredited Nurse Practitioner (NP) Program or Physician Assistant (PA) Program
  • Current Advanced Practice Registered Nurse (APRN) Licensure or Physician Assistant (PA) Licensure with unrestricted license in good standing
  • Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) or Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP) in addition to Adult/Family or Gerontology Nurse Practitioners (ACNP)
  • Active Prescriptive Authority in the state of professional licensure (unless prohibited by state regulations)
  • Solid computer skills, including Electronic Medical Records
  • Covers assigned geographic territory. May require substantial drive time in more rural areas

Preferred Qualifications:

  • 2+ years in practice (community or long-term care setting preferred)
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Experience working with patients in non-clinical settings
  • Advanced knowledge of and experience with symptom management
  • Understanding of Advanced Illness and end of life discussions
  • Awareness of health literacy and health equity in patient care settings
  • Ability to work with diverse care teams in a variety of settings
  • Effective time management and communication skills
