Position Highlights:

  • Practice with a multi-disciplinary team of heath care providers that includes Primary Care Physicians, RN Case Managers, social workers, and clinic staff
  • Our model is designed to provide the most efficient, comprehensive, and proven care techniques to treat the whole patient – physically, mentally, and socially – at each visit
  • Primarily manage care in the patient’s home; however, care may require an office or other facility setting depending on the needs of the patient
  • Provide care for some of our sickest patients – patients whose needs are beyond what our care management and clinic-based resources can provide, but whom hospice services are not appropriate
  • Diagnose and treats both acute and chronic conditions such as diabetes, hypertension, kidney disease and other chronic medical conditions
  • Treat acute conditions such as minor injuries and other acute medical conditions such as infections or exacerbations of chronic diseases
  • Facilitates patient and family decision making regarding treatment decisions, end of life care, and serious illness discussions
  • Prescribes pharmacological and non-pharmacological interventions

Required Qualifications:

  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of (NCCPA)
  • Current Licensure through the Texas Board of Nurse Examiners , or the ability to obtain one prior to employment
  • Valid DEA license or the ability to obtain one prior to employment
  • 1+ year of clinical experience as a Nurse Practitioner
  • Comfortable managing a primarily Medicare-aged patient population
  • Reliable transportation

Preferred Qualifications:

  • Home health experience
  • Bilingual (English/Spanish) fluency
  • A working understanding and support of a managed care environment
